List five words or short phrases that describe important parts of your identity. Choose one and explain why it matters to you.
Think about roles, interests, culture, personality traits, communities, or goals.
Sample answer: Five parts of my identity are student, older sibling, athlete, creative person, and friend. Being an older sibling matters to me because it has taught me to be patient and responsible. - 2
Name three values that are important to you. For each value, describe one action that shows that value in your daily life.
Values are beliefs that help guide your choices, such as fairness, courage, loyalty, learning, or respect.
Sample answer: One value I have is honesty, and I show it by telling the truth even when it is hard. Another value is kindness, and I show it by checking on friends. A third value is responsibility, and I show it by finishing my work on time. - 3
Describe a strength you have that other people might not always notice. Explain how you use that strength.
Sample answer: A strength people may not notice is that I am a careful listener. I use this strength when a friend needs support or when I am trying to understand directions before starting a task. - 4
Think about a time when you felt proud of yourself. Describe what happened and what it shows about who you are.
The moment can be big or small. It can involve school, family, work, sports, art, friendship, or personal growth.
Sample answer: I felt proud when I studied for a difficult test and improved my score. This shows that I can stay focused and keep trying even when something is challenging. - 5
Create a personal identity map. Put your name in the center and add at least six parts of your identity around it. Then explain which part feels most important right now.
Sample answer: My identity map includes student, friend, daughter, musician, bilingual speaker, and future nurse. The most important part right now is student because the choices I make in school are connected to my future goals. - 6
Identify one belief you have about yourself that helps you succeed. Explain how that belief affects your choices.
Helpful beliefs often sound like growth mindset statements, such as I can learn, I can ask for help, or my effort matters.
Sample answer: I believe that I can improve with practice. This belief helps me keep working after mistakes instead of giving up right away. - 7
Identify one belief you have about yourself that may hold you back. Rewrite it as a more balanced and supportive statement.
A balanced statement is honest, but it does not label you as stuck forever.
Sample answer: A belief that holds me back is I am bad at speaking in front of people. A more balanced statement is Public speaking is hard for me right now, but I can improve by practicing and preparing. - 8
Choose one role you have in your life, such as student, friend, family member, teammate, worker, or community member. Describe one responsibility and one challenge that come with that role.
Sample answer: As a student, one responsibility is to stay engaged and complete assignments. One challenge is managing time when I have several deadlines in the same week. - 9
Look at the idea of a values compass with four directions: family, learning, community, and personal goals. Choose the direction that guides you most right now and explain your choice.
There is no correct direction. Choose the one that connects most strongly to your current decisions.
Sample answer: Learning guides me most right now because I am trying to build skills that will help me after high school. I want my daily choices to move me closer to future opportunities. - 10
Describe a situation where you acted differently depending on who you were with. Explain why people may show different parts of themselves in different settings.
Sample answer: I am more serious at work than I am with close friends. People may show different parts of themselves because each setting has different expectations, comfort levels, and responsibilities. - 11
Write about one interest, hobby, or passion that helps express who you are. Explain what it reveals about your personality or values.
You can write about music, sports, gaming, reading, fashion, volunteering, building things, cooking, nature, or another interest.
Sample answer: Drawing helps express who I am because I like noticing details and creating something original. It shows that I value creativity and quiet time to think. - 12
Use a personal growth timeline. Identify one past experience that shaped you, one present quality you are developing, and one future goal you are working toward.
Sample answer: A past experience that shaped me was moving to a new school, which helped me become more adaptable. A present quality I am developing is confidence. A future goal I am working toward is graduating with a plan for college or career training. - 13
Describe one way your identity is influenced by a community you belong to. Explain how that community affects your choices, confidence, or sense of belonging.
A community can be based on family, culture, school, faith, neighborhood, language, activities, online spaces, or shared goals.
Sample answer: My soccer team influences my identity because it helps me see myself as disciplined and dependable. Being part of the team gives me confidence and reminds me that my effort affects others. - 14
Rate your self-awareness in three areas: emotions, strengths, and needs. Use a scale from 1 to 5 for each area, then explain one rating.
Sample answer: I would rate my awareness of emotions as 4, strengths as 3, and needs as 2. I rated needs as 2 because I do not always notice when I need rest or help until I feel overwhelmed. - 15
Write a short personal statement that begins with I am becoming someone who. Include at least two qualities or goals that reflect your identity and growth.
Focus on growth, not perfection. Include qualities you are practicing and goals you care about.
Sample answer: I am becoming someone who is more confident and responsible. I am learning to speak up for myself, take my goals seriously, and treat others with respect.
